I remember reading in several reviews that the Joker is a lot like the shark in Jaws; you're just as aware of his presence when he's offscreen as when he actually is, and that makes the build up so much better. And a lot of audience members are just so stupid used to being spoon-fed everything, and having every single detail laid out in front of them that they're incapable of getting the full effect of it.
